Overview
We are looking for a highly motivated and reliable NDIS Package Coordinator to be responsible for the coordination and provision of NDIS service agreements within the Eyre and Far North Local Health Network.
As the NDIS Package Coordinator, you will be responsible for ensuring that NDIS targets are met and that eligible clients receive appropriate services to assist with reaching their identified plan goals and to ensure that services offered are culturally appropriate and inclusive. You will monitor, promote and foster a culture centred on customer service and must provide a high level of empathy and support to clients and their families to ensure that service provision is efficient and effective within the NDIS Space.
You will work closely with Allied Health clinicians, Allied Health Assistants, specialist Nursing services and administration within Country Health Connect as well as other external service providers as per the client’s service plan.
